15. Do you have a permission slip for the parent and students to sign prior to using the Internet?

Yes, we do. We have an acceptable-use policy that states what they should not be doing on the Internet. That is discussed with the students. The students sign it. Parents are asked to read it and also sign that they have seen the acceptable-use policy. If students do not comply with the acceptable-use policy, then they are not allowed to use the computer that has access to the Internet.
